Name change requested by Papa Lopez to Freak Taco. Papa Lopez changed their trade name and the name of their LLC. According to TABC a new license was not required since their was no actual change to ownership within the LLC . Health processed name change request on H16-0224 only, but a new CO23-00138 was issued by Development Services. Supporting documents attached.
